The PERT Consortium™, in collaboration with leading experts, established the PE Center of Excellence Program to advance research-based best practices in pulmonary embolism (PE) management. As the accrediting body for quality PE care, we are dedicated to evaluating evidence-based practices, driving innovation through collaboration, and empowering healthcare professionals to enhance patient care and improve outcomes for all affected by PE.

How to Begin the PE CoE Application Process:

1. Review the PE CoE Criteria: Sites should review the criteria for both our Regional and Comprehensive Care Centers of Excellence. Please contact us if you have questions about which program is right for you.
2. Intent to Apply Form: Fill out and submit the intent to apply form here.
3. Application Fee: A non-refundable fee is required to begin the process.
4. Access to Application Portal: Sites will only gain access to the application portal after the application fee has been paid and all legal documents received.
