The PERT Consortium™, in collaboration with leading experts, established the PE Center of Excellence Program to advance research-based best practices in pulmonary embolism (PE) management. As the accrediting body for quality PE care, we are dedicated to evaluating evidence-based practices, driving innovation through collaboration, and empowering healthcare professionals to enhance patient care and improve outcomes for all affected by PE.

How to Begin the PE CoE Application Process:
- Review the PE CoE Criteria: Sites should review the criteria for both our Regional and Comprehensive Care Centers of Excellence. Please contact us if you have questions about which program is right for you.
- Intent to Apply Form: Fill out and submit the intent to apply form here.
- Application Fee: A non-refundable fee is required to begin the process.
- Access to Application Portal: Sites will only gain access to the application portal after the application fee has been paid and all legal documents received.
