Welcome to the PERT Consortium PRISM Program!

PRISM is a unique and transformative program that is designed to unify hospitals worldwide to provide consistent and the highest caliber of advanced Pulmonary Embolism (PE) care.

From initial team formation to full accreditation and continuous quality benchmarking, PRISM provides institutions with a structured, adaptable methodology rooted in collaboration based upon clinical evidence with measurable outcomes.

This form will help us determine your institution's eligibility to participate in this three-year program.

Prism Program Overview

The PRISM Program is a three-year program limited to 100 sites, (Regional and Comprehensive) that integrates accreditation with ongoing data-driven quality improvement.

Applications are accepted annually, and institutions may enter the program during any application cycle. The Prism program is designed to begin in May 2026 and conclude in April of 2029.

 

New Applicants (Entering Year 1)

  • +  Sites that apply and are accepted will participate in the full three-year program.
  • +  As part of this structure, the re-accreditation fee in Year 3 is waived.

Institutions Up for Re-Accreditation

  • +  Sites that are due for re-accreditation may participate in PRISM.
  • +   If they commit to entering data into the PRISM database, their re-accreditation fees will be waived.

Application Fee

  • + This fee applies specifically to the COE portion of the Prism program. A $2,500 non-refundable application fee is required for sites initiating the COE process; however, sites that have already paid this fee are not required to pay again. This fee is only required after a site has been accepted into the Prism program.
