Complete the enrollment form
Start by filling out the enrollment form below.
2.
Receive and pay invoice for the application fee
After submitting the form, your institution will receive an invoice for the nonrefundable application fee.
3.
Sign the mutual NDA and select a virtual site visit date
To gain access to the full application materials, you must sign a mutual non-disclosure agreement (MNDA) and a Certification Agreement. Once both legal documents are executed, your institution will have six months to complete the application. Please consider this timeline when you choose your virtual site visit date.
4.
Fee Payment Schedule
The nonrefundable application fee must be paid before the virtual site visit, and the accreditation fee must be paid in full before your institution can receive official notification of accreditation.
